This form is for adults who have delivered or supported a Nicofiends activity.

Please do not include names, personal details or individual experiences belonging to children or young people. If a safeguarding concern has been raised, follow your school, group or organisation’s safeguarding procedure.

ASH Wales will use your responses to evaluate and improve Nicofiends. Your email address is optional and will only be used to contact you about this feedback.
